End of life care at home

End of life care or palliative care as it is known is not a subject that many people like to discuss due to it's sensitive nature. There are many different services available to support a person at the end of their life. Providing this type of care at home provides comfort and dignity in familiar and safe surroundings for both individuals and family members.

What we do

1. A full assessment to fully understand the persons wishes and preferences during their journey with us.
2. Work with family and friends to ensure that they are supported and understand this difficult journey.
3. Liaise with other professionals to ensure that everything needed is in the home and available at the point of need.
4. Regularly assess the change in needs and adapt as is necessary.
5. Dignity and comfort is maintained throughout our journey together.
6. All of our team are trained to ensure that they know what to do and when.
7. Responsive to changing needs as is necessary.
8. Our management team will proactively monitor and pre-empt when extra support is needed and will discuss this ahead of the time.
